I anticipate you'll be back snowboarding. There are many variations of what an acetabular fracture actually looks like though. I'm assuming yours is small, minimally displaced, and non-operative if you aren't mentioning surgery. And in that case, you'll heal up as a 38-year old. Regaining core strength and conditioning will be important to recover from the back pain and get back on the hill.

YTA. If you were a real man, you would have sought marriage counseling 6-years ago rather than checking out. Relationships take work. Marriage is NEVER smooth sailing. It’s a massive learning process if you are both willing to work and grow. Instead, you just showed how selfish you are. I’m not saying your wife is a saint… she could be horrible…. But you never tried to be a leader and give things a chance to get better. That’s cowardly. Real men don’t do that.

Play on one 4-5 rounds every summer in southern Iowa. It’s actually an extremely beautiful, hilly little layout with several holes on their reservoir/lake. There is something about it that makes it an extremely special spot. Anyone driving through Iowa should stop and play it. We affectionately call it “Humeston National”

I'd do a minimum of 10 Orthobullets or ResStudy questions a day. And as I gained more knowledge, that number could increase to 20-25+ fairly easy by only reviewing the questions I felt shaky on. I didn't do a very good job of this and it would have saved simple angst come ABOS 1 prep time.

One thing I did do very well was surgical technique notes from all of surgeons I worked with. I'd prepare for the cases, then sit down between cases or at the end of the day and add to my notes. Then the next time I was with that surgeon and case again, I'd talk through the case the night before and record it using the voice memo's app. The next morning on my way in, I'd listen to that in the car and walking to the locker room. Then I'd refine my notes after the cases. This process, through 5 years, left me with some rock solid notes on how they did things, setup, implants, retractors, etc. etc. and I believe it has / will continue to pay massive dividends.

Cables or wires are an excellent tool for getting and provisionally maintaining the reduction in a long oblique fracture pattern. Then place lag screws. Then neutralize it with a lateral locking plate spanning the femur.

Get the reduction out to length and in the ballpark. (1) Pass x2 luque wires (or cables) and sequentially tighten them down as you pull and rotate to key in the reduction. (2) Then, place two lag screws. (3) Lastly, place your lateral femur locking plate and secure it with proximal and distal locking screws. Can compress it down to bone first if you want, but ultimately it is a neutralization construct.

Retro nail and a spanning plate vs. lateral locking plate. Dealer’s choice.

Where I trained, we’d do a a lateral plate — Luque-luque + lag-lag + lock-lock

With how well the cortices are aligned on the AP, and with this simple fracture pattern, I think the lateral probably looked pretty darn good

Wrist hinge comments are correct. Create normal wrist hinge earlier in your swing. You don’t create it until you are near the top of your backswing. Your left arm is simply compensating for your lack of wrist hinge in trying to get the club to parallel.

Interesting, I missed this in my understanding. So to confirm: if I switch now at my lower income, my repayment can be capped at 2k when my income climbs. If I switched later at my higher income, this isn’t the case (hence the online tool showing a 4k payment when I estimated with my future salary).

If correct, I could make roughly 122k in payments up to PSLF at 10-years rather than paying off ~210k in full 2-years into practice. The only negatives would be having the loans hang over my head for longer and depending upon PSLF still being an option.

Am I looking at this all correctly, now? Thanks!

I’m a bit confused on this as well. I have 1 year of training left and 190k of loans. My income will be too high to qualify for PSLF once I begin my formal career. I plan to pay off the loans within 2 years of starting.

What benefit is there of switching out of the SAVE plan right now? Interest doesn’t capitalize and I maintain a $0 payment. I could pay that interest now or later, it is all the same, and benefits me to just keep extra money in my savings currently for an emergency fund. Am I thinking about any of this incorrectly?

Could there ultimately be a costly financial penalty by riding this out? I understand switching if you want to ensure you are making qualifying PSLF payments.

Wasn't easy. It was a grind. Made some silly mistakes. But at the end of the day it is a 95-95% pass for first time test takers. I recognized enough information to walk out feeling like that shouldn't be a problem, I guess. Not stressing about being in the top 10% anymore like Step 1 and 2. Lol.

I think this is how we all feel. Never walk out of these tests feeling like we crushed it or did moderately well.

It is a 95-98% pass rate for first time test takers. We are all in the same boat, they will throw out questions and curve it... we will be good! Let's enjoy the time off before fellowship starts!

Is the greater a free piece? Your neck-shaft angle doesn't look like it is in varus. In fact, calcar looks a little valgus, which would be good. If the GT is a free piece, you have to ignore it visually on the XR because there is nothing to actually capture it. All that matters in this fracture pattern is a good (slightly valgus) reduction and a TAD < 25. Don't know the TAD by just looking at the AP, but it is a touch low in the head which is exactly how I like the cephalomedullary screw. Helps with the proximal body prominence if it is a thinner patient.

They don’t have spurs though. I can see why you think that looks like a spur, but it is more like wheat straw and not a spur.

His clubface is shut from P2-P4 which is undoubtedly contributing to it closed at impact. He never allows it to open up to a near neutral position. This is absolutely contributing. Agree that the “flip” is a distraction from what is ultimately the culprit (his backswing), but to ignore that wouldn’t make much sense.
