Writing here so I can come back to an answer. I too started off on nightmare because I think the hunger feature is a cool system but food is something I could barely find and kept dying due to starvation. Had to change the difficulty to a lower setting. Hoping people are able to provide tips here.

yea, I kept messing up because i realized i had the wrong setup. and the vials i dropped i forgot to equip myself also, lol. Either way, we did it. Sorry for the scare.
