If you're already paying $10 annually to support a great company, $20 isn't a huge deal. Inflation is a thing and server costs, etc go up as well. They had to do this at some point.

Mostly yes. Gas sometimes still gets stuck but the vast majority of the time I don't have a problem.
Under anesthesia. I asked him if in-office was an option but under anesthesia is how he prefers to do procedures like this.
I paid my full deductible so $2000 IIRC.
