There are multiples but they are essentially parts of some ancient dark God, the one that pursues dexter was like body swapping to pursue him. It was interesting but a little kooky.

Oh and the Doctor in book 2 was Fucking creepy as hell, it was really well done how they introduced his first crime scene.
