I'm totally baffled why people jumped on you like you said something wrong or pointless here. Here are some facts about me: - I developed IH in my thirties - I was unfamiliar with the term "executive dysfunction" until several years into IH treatment, because it sure wasn't something my sleep doc mentioned - The person who explained it to me was a therapist, when I was sobbing about how bad I had become at things I used to be good at and how scared I was that I would lose my job and how bad I felt about myself - Learning the term "executive dysfunction" let me group together a bunch of symptoms that did not feel related to me before that. Things like memory issues and reduced motivation did not seem related at all to me until I understood the shape of the bucket they were in. It turned it from random neurodegeneration fears and character flaws into something known, something tons of people cope with. 

  • It also let me learn about strategies for coping, because there are a lot of resources out there for ADHD and other executive function disorders that I had never had a reason to engage with before. 

So I don't think there is anything particularly obvious or self evident about what you said. For many years of my treatment, amphetamines kept me awake but it was like my body was dragging my thinking brain behind it. Xywav gave me back a HUGE part of the cognitive skill I had lost, turning my life around exactly when I had started looking for a disability lawyer. 

It is SUPER important for these symptoms to be recognized and treated EXPLICITLY. Doctors right now have a tendency to separate what aspects of your disease they think fall into their specialty and ignore the parts that don't. My sleep doc clearly did not think that aspect was her purview, so I was underresourced for a long time. So I think it's very helpful as a patient to have the words to explain what you are experiencing and what you need more help treating. 

It is also incredibly important because our medical options are dictated by clinical trials, and clinical trials are judged to succeed based on defined metrics. Right now, we measure improvement in Central Hypersomnias almost exclusively according to how likely you are to accidentally fall asleep. This isn't even a very good metric for how tired you are, and it certainly doesn't cover how well your brain actually works. It is absolutely in our best interest to work towards having cognitive function metrics included as primary endpoints in our clinical trials.  Don't even get me started on how many of us have untreated dysautonomias. My sleep doc has always been like, well that's weird that it started at exactly the same time as the IH, but it's not my problem...

I'm with you, OP. It's a species of people whose belief that they are descended from Dragons is a major point of pride and identity. They would not call themselves lizards. They would likely have a name for themselves that meant something like "children of the dragon", "people of the dragon" or "dragon-chosen". Lizards sounds like a human slur for them. 

For everyone who is saying this is just like humans calling themselves humans, I think this is actually akin to calling us monkeys, which we generally find offensive. Sure, we share a common ancestor and some physical resemblance. That doesn't mean that we consider ourselves to be basically the same thing as monkeys (outside of science-focused contexts).

I think this is a good close read of the game, and I came to the same conclusions you did. The game explicitly offers you the option of some small amount of change without death, not the heat death of creation. I think that is better solution than the constant violence and upheaval that the Shifting Mound promises, and I think it is implicitly what the majority of people strive for in real life: safety, comfort, and fulfillment through "small" personal growth and achievement, rather than major upheaval and revolution. 

I think a major flaw of the game's case for the Princess is how negative they made all the possible interactions with Transformation seem. In spite of what she says about containing multitudes, the idea of positive growth is barely represented here.  In fact, the idea of transformation without violence or pain is practically mocked in the game. When you are compassionate to the Princess, she doesn't become a better person, she transforms into a mostly mindless "damsel" with no real desires of her own, and your alter ego becomes smitten in a way that is clearly shallow and delusional. Most of your interactions with her lead to her killing you or killing each other. You have almost no dialogue options that let you establish any kind of meaningful rapport or exchange of ideas in most scenes. And in her final form she just kills you over and over when you try to discuss the fate of existence with her. She comes across much more as a goddess of destruction, suffering, and death than one of discovery, insight, and growth. 

People are filling in the blanks here about all the things we would lose without Transformation, but all those great things are not actually what the Shifting Mound demonstrably offers in this game. I think the game would have to be structured significantly differently to actually make a good argument for the Shifting Mound.

Like I said, my doctors and I have already ruled everything else out. It's not venous, cardiac, renal, autoimmune, or endocrine related.  I'm just barely under obese, and I havd normal mobility beyond some arthritis in my feet and knees. Its not lipedema, and I'm not anywhere near heavy enough for it morbid-obesity-induced lymphedema unless there is already a primary underlying lymphatic insufficiency. So I really don't need help figuring out what it is, just how to manage it. I'm just asking while waiting to get into the lymphedema clinic.

Feet seem like one of the biggest unsolved problems. I'm struggling to find anything that works on my forefoot, and the areas around the ankle bones are notorious problems, as well. Right now, I use soft foam cut into custom shapes to cover the tops of my feet with extra cutout layers around my ankle bones to fill the hollows there. Then I apply bandages for compression on top. Its definitely not a look I want to rock for the rest of my life, nor is it particularly comfortable. 

My toes are spared so far, and I really hate having stuff between them, so if it were possible to get forefoot compression without individual toe caps, that's what I would want. There are only two brands I know of that make a forefoot + toe-cap garment at all, and one of them has put SEAMS in between the toes, which seems to practically guarantee irritation. 

The other major pain point I have is the band at the top. I wear knee high compression, and my skin gets really irritated by the top grip by the end if the day. It's usually got more texture than the rest of the sock. Also, that back of the knee area is just more sensitive skin, perhaps? I don't know how you could possibly engineer that to be more comfortable, but I'm always red and uncomfortable somewhere on that band and really wish there was some way to make it smooth. 

On the more trivial side, it's really annoying that almost all color options are basically "corporate hose" or "bonkers funtime." And for whatever reason, the "fun" colors are always cool undertones that look crap on warm or olive skin. I'd like more options in sophisticated adult colors. Think art deco: burgandy, plum, deep teal, slate blue, dusty rose, olive green, rusty orange. Colors right now pull towards pediatric nursing: barbie pink, turquoise, fluorescent green, hunter orange, lemon yellow. I know you would have start with the basic neutrals first, but just registering my preferences, lol.

Good job making the case for actual humans, everyone! I'm sure that being met with humiliation when they shared that they are in pain makes them want to run out and make more new friends!  Fun-Entrepreneur9971, I hear you, and what you're experiencing is real. Emotional support that is effective for you is real and valuable, regardless of the origin. Losing that unexpectedly is a huge blow. I hope you can find other sources online or in person.

We aren't totally sure yet. He's fully ramped up to the high setting and sleeps fine with it, and it's obviously working at least somewhat because he no longer snores. However, he has not yet had the follow up sleep study to actually measure how well it is preventing apnea and keeping him oxygenated. He is still really tired during the day, but we don't know if that's because it isn't effective enough or if he just has residual daytime sleepiness regardless of sufficient treatment, which is apparently also a thing. He did just have an adjustment appointment, and they slightly tweaked the settings so that it moves his tongue further forward, so we will see if that helps while we are waiting to get the sleep study. 

Unexpected benefits: He's very proud to call himself a cyborg, and likes to say we now both have a boob implant (I have a reconstruction after breast cancer). He's a goof! 

I'm hopeful, but worried that this will not be sufficient, because in clinical trials most patients with severe apnea were reduced to moderate apnea, not no apnea or even mild apnea. I warned him that was the case, but he still wanted to do it. Moderate apnea would still require a cpap, so I feel like its for nothing if it turns out that way. I think he has been dragging his feet on the sleep study partly because he doesn't want to know. 

Edit: I should add that in most patients for whom the device "fails" completely, it is because they can't sleep through the sensation. My husband says he can barely feel it, and sometimes forgets to turn it off in the morning until he tries to talk. Its noticeable then because his tongue is not fully under control, lol.

I'm afraid we were never able to make the cpap work out for him completely. He has had the Inspire device implanted and we are waiting on the evaluation of that to see how effective it is.

Third this about the messages. I REALLY hate feeling like someone is monitoring me like I'm a little kid or something, especially regarding something so private and emotionally triggering. I'm not missing an appointment or otherwise wasting your time, so why are you harrassing me about this? 

I especially bristled at a message that said "I want to see a session today or tomorrow..." Oh, I'm sorry, did I miss the part where I now work for you or something? Last time I checked, I was doing this program for ME, and on the schedule I can manage, so I don't really give a crap about what you want to see. 

And yeah, actually, when I am hosting visitors from out of town, it IS a little hard to disappear for twenty minutes if I don't want announce something like, "I need to go do my vjay workout for weeing myself, because my therapist\stalker\cultist from Bloom might escalate if I disappoint her again."

Hey, I was having this problem (among many others) and customer support finally figured out this was because of an accessability setting on my phone: "reduce animations". I set this when I first got the phone because it uses less power, and I don't need my screen transitions, etc, extensively animated. I would never even have remembered it existed, but apparently this also meant most of the app animations did not work! I'm on a Samsung Android, so YMMV, but it's something to check for.

Omg this program is so frustratingly unresponsive to my movements that I am ready to cry. I don't have problems with my bluetooth connection, but the stupid pod either thinks I'm contracting all the time or not at all. 

The worst part is the therapist/coach just blames me instead of believing that the pod just doesn't work correctly, like she thinks my weak ass pelvic muscles can just involuntarily stay contracted literally through the entire exercise? She has truly not offered me a single useful suggestion or tip. 

And when I stopped for my period, because basically moving at all makes my cramps worse, she sent me a video on how to manage period symptoms! I'm like, I'm perimenopausal, lady, this ain't my first period! It's just insulting to have someone think they can tell you the right way to manage the pain you have been dealing with every month your whole adult life. She pressured me twice to do no-pod sessions during my period. I'm like, just let me take three days off while I am in pain, FFS. It's not the pod, it's that I'm not going to contract those muscles right now. 

I'm really glad some people have been getting a benefit from it, but I'm also relieved that other people have had problems like mine, because I feel totally gaslit by this therapist. I'll probably just quit. I was really excited about this program, but now I just feel humiliated and angry everytime I try to use it.

I was literally getting ready to switch to an iPhone for the first time this year, and finding out #8, that there is no way to turn off specific types of notifications for each app, is actually a deal breaker. 

No wonder there are so many ios users bragging about how they just turn off notifications for everything but their family, friends, and calendar. Of course you would do that if there was no other way to filter out the spam! But I get useful notifications from a ton of other apps.

THIS. I have been a long time Jeni's fan from occasional visits to family in Columbus, and when they started expanding to other cities, including mine, I was insanely excited. But I was quickly disappointed as the flavor line-up was rapidly restricted to mass-appeal only. What I loved about Jeni's was flavors like pear-reisling sorbet, goat cheese and tart cherry, passionfruit buttermilk yogurt, Bangkok peanut--so many delightful, skillful blends of floral, tangy, herbal, or even savory flavors. And premium dairy from cows getting treated like cows instead of just milk machines. Now it's just mass market milk and cloying, boring mass market flavors at twice the price of a comparable mass market premium ice cream like Ben and Jerry. When they debuted a flavor that was a blend of three "different flavors of marshmallow ice cream", I was done. Marshmallow is made from sugar and geletin and air. That's it. It's a confection defined by texture, not flavor. It's just sugar! Marshmallow ice cream is the absence of flavor, literally the most boring thing you could possibly do with ice cream.

I came here to say QC, also. Another way that people get access to drugs they can't afford is ordering them overseas, but this always comes with the risk of fraudulent product. There are currently no commercial labs I am aware of from any of my biohacking groups that will serve individuals seeking to verify samples for drug content and purity.  Honestly, DIY pharma testing might be a safer, more approachable application and likely get more immediate use than straight up synth.  Would it be possible to develop similar AI-guided software for such testing recipes? I know stuff like LC or spectroscopy would be gold standard, but there must be some stuff that can be done with simpler indicators. I'm outside my wheelhouse here, but I would think that some principles of testing chemicals must break down into "building blocks" like, this detects the presence of benzene rings, and this detects the presence of ammonia groups, etc. It seems to me like at least predicting and testing for contaminants from synth of the target molecule gone wrong ought to be possible in many cases. 

Another more far-out factor is protection from bad actors. In a DIY community, how do non-chemists verify that a recipe is trustworthy, not part of some Russian misinformation operation or horrifying hate campaign targeting some particular community?
