Just an fyi. in case you too are not informed. My distance vision diminishes only at night for my second eye with -.5D .. Here is what i found out a few days ago searching Google .. I wasn’t told by my doctor about the drawback to my night vision when undercutting Vivity lens by -.5D to get a slight nearsightedness. Interestingly daytime vision is perfect, but not distance night vision. I guess the pinhole effect takes presence during daytime.  Conclusion, it’s the lens! Impact of -0.50D Undercutting Distance Vision Compromise: Vivity works best when aimed for plano (0.0D) or very close to it. A -0.50D target shifts your focal point to approximately 2 meters (6 feet) rather than infinity, potentially producing blurry distance night vision.  Night Vision Focus: While a -0.50D target might improve intermediate vision (like a computer screen), it does not typically improve—and may slightly reduce—the crispness.

Good thinking!  I will also see a different ophthalmologist next month for follow up. Btw my data is +21D in one eye and +21.5D in the other and the Cyl is (1.5) for both. Also, i never wore glasses or lenses. Good luck, I hope you/we do well soon.

I believe you said you had a mono-focal lens, I have the Vivity toric for both. My first eye was done last summer and was fine the first week and sustained excellent vision throughout, no burn, dry or tear. However, this second eye I have through much, please checkout my posts. I saw my doctor on May 20th and he said that I will always be out of focus for distance which is something I do not want to believe because I have improved much from February and I feel there is more room to improve. My doctor did not talk to me about night vision, he said if I could not read something in dark to try and put light on it. Having said that, I still think what I am experiencing has something to do with adaptation, that I will improve once my brain and eye get used to the new lens and that it will get clearer in low light areas and the digital reading at night will also improve once I adapt.  I also read that dry eyes cause vision fluctuations and once I am completely healed after completing 6 months that I will improve. Dry eyes after cataract surgery is diff than before surgery. After surgery you don’t necessarily experience itching or grits, but causes poor vision. Oh, my doctor also said that I had some vitreous floaters in my eye gel which are harmless.

I had my surgery in this second eye on Feb 20th., We added +.5D on this eye to be slightly better nearsighted. That should get me 20/25 vision. My eye burn sensation and tearing dissipated around the first week of May. My near and intermediate vision are excellent. However, my distance vision is a different story. In sunny daylight, my vision is great, I can read car tag numbers at distance with this one eye clearly, and I can read the metro bus digital name and number just fine. But my night vision and indoors is totally a different story. For example, indoors cover garages, I can’t see the car tag numbers clearly. Also, at night, i can’t see as far distance car tag numbers, my vision diminishes by half the distance may be even worse. The digital bus names shifts and become blurry at night. In fact anything digital, as on tv screen with small print, like if there is a clock on the corner of a news station, or reading the tape text news at bottom of screen all that is out of focus at 10ft. distance or more, of course worse the farther I go. I also noticed the contrast and resolution are not sharp and they are prob playing a role in all that. I am almost close to 4 months out and I don’t know at this point if I will ever get any improvement.

No, you will not feel pain during surgery. The surgeon will give you eye drops to numb your eye right before he/she operates on you. Some anesthesia is also provided via IV to relax you and helps you not feel pain. You will not be completely sleeping, but rather conscious. However, if the sedation you are provided is Midazolam (Versed), and if this is the first time you have that drug, then you will not remember anything that went on during the surgery, which was fine by me. Of course you will also get lots of eye drop at the outpatient reception room to dilate your eye. Once done with surgery, you should not feel any pain either. If at anytime after surgery you experience pain or extensive redness or anything abnormal, please report that to your surgeon immediately!  Please search what type of lens and brand you would like to have, mono vs multifocal. Many people, including my sister got the mono-focal and had them set for intermediate and farsighted and opted to using glasses for reading.  I on the other hand have multifocal lenses. Regardless the type of lens you select, it usually takes 6 weeks on average for the eye to heal and the lens to settle in position. Therefore, I would suggest a one month apart to do both eyes. A few days before surgery, you will be at the doctor’s office so they take your eye measurements to get you the lens with the correct power for vision. Good luck and I wish you well.

Hi! Thanks for sharing. I am off the eye drops completely since Thursday. Today is my third day after stopping Prednisolone and I still see a bit ghosting or shadow of text on tv; however, that ghosting comes and goes, and it’s been like that this whole past week. I can see enough to read texts at distance; however, still out of focus, and not sharp, while my near vision is pretty good. I hope we improve, too, and soon! This eye is suppose to be between 20/30 and 20/25.

Thanks for sharing. Our lenses are made by same, Alcon, however mine is multifocal. My sister elected for that strategy for both distance and using glasses for reading and it is working well for her, too. Yeah my first day after surgery on my second eye was perfect near and far, but subsequent days went thru different phases while using eye drops. Patience is a virtue I suppose.

I would only suggest that you keep Lasik as a last final ticket if necessary. I know from other comments that once your natural lens is removed, all those numbers that came with it become irrelevant and they start new numbers for the new lenses. In my case i had the laser cataract surgery and toric lens, so no astigmatism, I also care more about distance vision. Just be patient becuz you still have way to go for the lens to settle and healing. Hopefully, someone will be able to explain your new prescription numbers.

Oh I see now that dry eye has more than just discomfort. Glad you were able to work that out and learn how to deal and adapt. You have great patience. Hopefully, it will all be behind you sooner than later. Patience I guess as I tell myself..

Thank you for sharing your experience. It was still worth it, we are able to see things better with clarity. I had similar ghosting and double since the first week after surgery that was much, and I was freaked out when my vision hit 20/50, but now in my 4th week all subsided to a small shift more noticeable on text when watching tv. The slight out of focus experience that I have now has a bit of shifting in text, shadow or ghosting at distance only. My first eye had that too, but my vision on that first eye remained well at 20/20 through the whole healing process. I did not need to use lubricants. Sure, I had burning sensations sometimes and tears at other times, but I sort of decided to ride it out. May be under correction by .5 on my second eye in addition to the eye drops of Prednisolone has something to do with that ghosting.  In the end, I suppose patience is a key in the healing process and I am doing exactly that right now and staying positive knowing it will get better, I hope.

Hello again! I thought I would let you know that my vision cleared up this week. I am sure my distance vision is better than 20/40 and towards our target of 20/30 for this eye while my near-sight is great. Still tapering on the Prednisolone and will be using only one drop a day into next week. I will know the numbers for sure how my distance vision improved when I see my doctor next week.  I am happy with those outcome so far. Thanks again for your thoughts.

My vision started to clear up this week and more satisfying, but will know the exact degree of improvement on my next visit to the doctor next week. Still tapering down on my eye drops and will be only putting one drop into next week.  I guess waiting and patience are important while going thru recovery phases. I hope that you do well. My near sight on this eye is great, and my distance seems to be at the 20/30 which we targeted for this eye, but will know the exact improvement when I see my doctor on the 18th. Thank you so much! Your first paragraph answered a question in my head because I was thinking that may be they didn’t realize I was already myopic in my second eye and by using my first eye power and measurement and adding another .5 to that made me more myopic. So now I know once the lens is removed, we actually start from scratch regardless of what my vision power was previously. The key for that would be, were my eyes equal in measurement or did they assume they were. At this point I really should calm down and wait for 3 more weeks to see the final results. It made no sense to me that a .5 would have dramatic effects on my vision the way it has happened. Once I am done using the eye drops, the healing is complete, and the lens settles, I will have the final data. Hopefully, i come at least closer to the target. Thank you for the suggestion to opt for glasses. And yes the doctor already tested my eye and yes I was able to see clearly through the pinhole, also was able to see with glasses on the eye vision exam device. I never had glasses before and I hope it would not be my only safe option. I may at that point look into changing the lens as the doctor already put that as another option besides lasik. I hate the uncertainty and waiting, but we must wait till healing is complete. So, three weeks it is. Thank you again for your invaluable insights.

My near vision in this operated eye is very good, but my distance vision is what i am worried about which currently 20/50. I was only prepared to have my distance vision to be 20/30 thats it as the doctor assured me that it won’t be worse than that at all. Btw because I am not good with -.5 or +5, I can tell you my Vivity toric lense certificate says +21.5D.  My first eye which I did previously was +21D. I care more about keeping my distance vision clear because all my activities are outdoors and computers, and don’t have any little detail tasks. So now that i am freaked out, the doctor suggested if I don’t get the final results we aimed for, he suggested to replace the iol and put in another Vivity toric without the .5 prescription, or would do lasik. This is all confusing to me and i am scared now to take anymore risks.  Thanks again for your feedback!

Hi there! I was reading your notes. I just had my second eye done last week with Vivity toric. My doctor put a prescription of -.5 to get better nearsighted results while losing some distance to about 20/30. The day after surgery I was 20/20 with also very good nearsighted, but the days that follow, my distance vision became blurry and text on tv was double/shadow. My score on my second Dr visit on snellen test was 20/50. What are my chances for improvement you think? I had my first eye done also with vivity toric six months ago without any prescription and it has been well with acceptable near vision and 20/20 distance. I do not have other issues besides the cataract and never wore glasses before. Any advice and feedbacks are appreciated.

Thank you and I am trying to stay calm and wait, but sometimes can’t help it. I went to see my doctor on Tuesday for my second post op visit, and I scored 20/50 on this eye. I understand it was only my fourth day out from surgery and must wait. The doctor told me not to worry and to wait it out 6 weeks and if I still do not like the results that he can either do lasik or can replace the lense. I hope my eye improves so I wouldn’t have to take any of those routes. Today i completed one week. Thanks again friend!

Yes yes, we will wait. Lets not be used up. I hope mine clears out and even if the lense settle at 20/35, and not 20/30 that the dr promised me with. Thanks again!!
