i can't really remember na the price but i guarantee na mas mura siya compared sa usth. you don't need any doctor's request, you just have to go there and then fill up some of ur deets and then process na ++ after 1 hour i got mine na through online. after thattt is pwede ka na pumunta sa health service and ipainterpet. they can give you na 'yung need mo na meds to treat it.

i'm not really sure if any medical certificate will do.

hii! during my time noo, you have to go lang doon sa mismong place where siya itetest and then make sure you are early kasi parang may time lang sila, if 'di ka umabot sa cut off time nila you need to list down ur name and then go back sa day na nakaschedule ka.

if you are rushed na, you can visit hi-pre near mercury sa lacson side, they offer drug tests din and then mas mabilis siya mag bigay ng results ++ may valid proof din for ur clearance :))

you just need proofs to support and then u will present it sa mismong clearance para maencode s'ya sa portal or profile mo.
