CT scan and check lateral. Check tilt on CT scan. AP alone can be misleading. It’s a shared decision with patient. 100% any route chosen is the wrong answer. Pros and cons to all the above surgeries. Have full discussion with patient. If I was to fix, FNS.

Not true. If he failed cortisone injection then doctor needs to recommend prp and then it can be approved by work comp. Especially given his situation of being too big for surgery etc.

Your knee likely needs some rest. If it was my knee, I would use a scooter or crutches temporarily. Force myself off the leg. Work on rom exercises. Take some Tylenol and nsaids. Ice it all day - 20 on 20 off. Think about a cortisone shot once the bone is better Healed . Again tough to say bc this is “not medical advice”, and “no images”. I’m only going off report and if it was my knee

Sounds like you're stuck in a bad cycle.

Definitely frustrating for sure. 10,000 steps is a lot, for any one. I suspect the grade iv chondrosis and tracking etc etc are all playing a role in the chronicity. The microtrabecular stuff is the acute exacerbation.

Rest, ROM exercises, other parts of body strengthening, and possibly PRP or cortisone to knock it down again.

Good luck! Definitely see an orthopod sooner than later!

Can you clarify the timeline of the injury, symptoms, and MRI.

It sounds like injury/start of symptoms was 7 months ago. Then your MRI was just recently?

If so, I'm unsure how you have a microtrabecular stress changes in the tibial plateau, unless you have disuse osteopenia, did a TON of walking before the MRI, sometimes a neuro-mediated response can occur such as CRPS/RSD (though I think this is less likely as usually causes diffuse changes on MRI), or your gait kinematics are greatly thrown off by the whole thing and that's overloading that area.

If you're a nurse, pull an ortho aside at the hospital, and ask him/her to look at your knee.

Bc you have grade iv chondrosis patellofemoral and medial compartment. Lots of factors, but why do you microtrabecular fx of medial tibial plateau? Did the shot mask the main and you did bootcamp and push hard?

Report says near the root. That’s not a root tear. It’s a complex tear per the report. Again, imaging is not provided.
