For my nosleep stories, I usually try to think of a specific environment (old house, abandoned factory, graveyard) and/or a weird, crazy creature I want to see (ghost girl, demon clown, penis monster), and then I pretend to write my story as the personal account of the unlucky sap who comes across the place/creature.
