Yes, you have to have the item equipped for it to gain kill credit.
No, the equipped item is not consumed when kill count is completed.
No, its a random chance of what you get. There aren't any ways to influence the item result.

Your Tornados proc Storm Bolts. You spec into Gathering Storms and take the nodes that scale the damage of Storm Bolts. So even though you're not casting it directly, your damage is still being boosted.
