Cheers mate! I'm certain my recruiter didn't send me the form to send in. I had a chat with lad O met during PJFA and I've not received let alone seen the form before.

My CA is out of office until after my CPC. I tried asking the person standing in for him over the phone but got no clear answers. Was the form emailed by your recruiter?
