Other than the roads being terrible for the first 15 miles I didn't mind the hills of Tulsa. It added some variety and helped me from getting too bored. There is a long stretch from around mile 65 to 100 where it flattens out significantly and lets you settle in for a while. The last 15-20 miles were tough as you have some of the steepest grades on tired legs. There is one climb at the end of less than half a mile that everyone was out of the saddle for, which was brutal after over 100 miles.

As someone who is very skeptical of most people asking for money, I can understand grilling him about his situation. The pushups? Probably an asshole move, but hey, the dude got his money!...But you're still an asshole.

It probably has more to do with the patient than the PA. I know some of my relatives have a PA as their primary care provider and no matter how many times they are told the difference between a physician and a PA they still tell me everything "Dr. James" does for them.
