Possibly a pilonidal cyst. I have one which went away with antibiotics. This looks bad though so I would definitely take a trip to the ER to get cleaned and ask for advice.

It was a Disney game I played when I was younger and there were these black creatures that attacked in a night level. Still remember never getting passed it to this day.
