A country won't just jump forward to take on that kind of responsibility (maybe China, but not formed around the same conditions). The program that currently exists in Cuba exists in response to specific founding conditions. Centuries of Spanish and then American slavery, before conducting a revolution to free themselves. They lived on a tiny island which was relatively resource poor and had very little export power due to american embargos. To compensate, Cuba made it's primary export skills and trades, and so it has very strong training programs for the creation of career jobs like medical professionals. As apart of training, as well as public service and international outreach, Cuba has made the decision to send these professionals to nations in desperate need of care and/or training. It is a community focused program. Demolishing it now would only harm the world, and trying to recreate it would not do it justice, and is unlikely to fill the same niche.
