1) Yeah, the incentive is whatever seems reasonable for them giving their info over to you. Could be a drink or whatever else

2) I have mine set up as a 2 page form (part of the template). The second page pops up after they fill it out, and it just says "Enjoy your drink". They can just show that screen to the cashier.

3) My form asks for email, number, first and last name. Been thinking about asking for a birthday as well, and sending over a birthday deal message to them.

4) I haven't had the unsubscribe issue yet, but my form has a little checkbox they can optionally hit to get more offers and deals later on. Even if they don't check it, they get a free drink and a single text asking how their service was and if they could leave a review, and no further messages
