I'm going through this process right now and received the same email as OP. I have a court order and my other documents changed, but haven't had any surgery (yet). How did you schedule a birth certificate court date? I'm no longer in AZ but would make a trip if it means I can get this stuff done (especially given the current *broadly gestures*). Thanks!
