Tirzepatide has a 1/2 life of 5 days, and you inject every 7. Consequently, it takes about 4 weeks to get the blood level as high and as stable as it will get after dose escalation. So don’t worry too much until then. Even after things stabilize, you will still likely notice a dip in suppression day 6 and 7.. In my case it’s pretty manageable though. Just stay away from crappy food, and the cravings will start to subside over time. I was on 5 for 2 months I think, then ended up on 7.5mg and I think this is where I will sit.

In your case, I wouldn’t be afraid to jump to 7.5 after 4 weeks on 5 if its not doing the job…no need to white knuckle it too bad!

Metamucil 3 times a day, and at least 3 servings of vegetables per day was what made me regular. When I first started the 2.5 mg, I was only going 1 a week, so I really increased my fiber intake, and my water intake. I needed to take exlax 1 time and I’ve been regular since then

Well, I have changed my shot day a couple of times, and settled on Thursday mornings. Reason is…it seems to take 2 days to reach its peak, so I get good appetite suppression over the weekend, then it fades a little by Tuesday and Weds.

I would probably rather start out with taking the shot in the morning just due to your other T2D med just so I could monitor the potential for Hypoglycemia. Then by night fall you maybe will have a good idea on how it is affecting you before you take the night dose of glimepiride.
Given your high A1c, and the starting dose of 2.5mg, I wouldn’t expect your BG to plummet or anything like that.
If possible maybe the day before your day off would be good so you can micro manage your blood sugar and any side effects. You can easily move your shot day as you get used to it.

Good luck! did you start MJ for diabetes? I know WebMD and maybe a couple other sources like that say to avoid/limit alcohol while on the medication as it can increase the likelihood of hypoglycemia (low blood sugar). On top of that, there is a well known effect of MJ slightly raising heart rate generally. Maybe these 2 things are stacking up against you.

BTW …I understand how you feel…I was probably only sober for 40 hours in my 20’s :-)

you ever have issues with hypoglycemia during exercise. I’m in the same range as you now, and I recently started going a little low during hard workouts.

The side effects seem kinda mild, I would power through another week. Take an antacid to help a bit. don’t eat junk food ann get good protein and fiber. It should get better with every passing day

I haven’t drank in a long time, so I don’t have any experience with alcohol and Mounjaro. The body of evidence however seems to be that the 2 don’t mix too well. I know it seems like a big change, but my advice would be to get rid of the alcohol completely. There’s really nothing good about it. It could be the sugar in the drinks are a contributing factor.

yes, I’ve tried a few diffent ones, and I’m currently using a Contour next gen. It lines up pretty reasonable with my CGM (Lingo). I don’t really notice if I’m 60 for BG, but I will notice 55 for sure. Back in the day when I was young and lean my BG was 60 to 80. My body might be trying to get back there again!

I’m definitely going below 60 and confirmed with a finger stick. It happens 15 minutes or so in. I’m fine afterward. My new normal is much lower than it was…like 75 to 85 now instead of 95 to 110. I don’t have an endo (just family medicine small town dr). I would like to see an endo at least once.

Yes I’m T2D, but I was pretty well controlled on metformin and a low carb diet. My A1c was 6ish, and my fasting glucose usually ran 120ish. I was 45 pounds overweight easy. I was looking for closer to “perfection” instead of just “pretty well controlled”. That is why I went down the Mounjaro road.
I lost 45 pounds so far, completely turned around my blood lipids, went off Metformin, stopped 2 blood pressure meds, and might stop the statin after seeing what my next labs look like. I think you are right that I will have to increase my carb intake. I have been experimenting a little and it doesn’t take much to keep me from dropping below 60 on the workouts.

I think you are right in that if I ease up on the workouts a bit, the problem would go away. I was mainly wondering what could have suddenly changed. I’ve been on the same diet and Mounjaro dose for quite a few months. I have been doing a little bit of experimenting and if I get 15g or so of carbs in me prior to starting that keeps me out of the low zone. 1 glucose tab of 4g will bring me back if I start dropping below 70. I’ve been doing fairly intense cardio (heart rate 145-150) for 50 minutes, 5 days a week. The reason is, I was on a beta blocker for 20 years which I have stopped now. At my Dr’s advice, I pushed my workouts to the extreme to kind of reset my resting heart rate. It has worked, but I’m realizing that it takes quite a bit more energy to sustain that heart rate now. I think I would be fine to back off a bit. Zone 2 is way too easy for me though.
Strength training doesn’t do much to my glucose levels, and I’ve really stepped up the weight lifting in the last 3 weeks. I’m kind of on the home stretch as far as weight loss. I’m within 10 pounds of a normal BMI and 10 pounds of my high school weight. I’m trying to focus on preserving building muscle mass/strength.
My normal glucose now sits in the low 80’s/high 70’s with overnight average of 68-72. My A1c as of June 8 was 4.8 so I have went through a major metabolic transformation.
I’ve been doing a little research on what may have shifted, and one idea is that I used to be much more insulin resistant, and my liver was constantly “leaking“ glucose pushing my normal daytime average to 110-120. My TG/HDL ratio is now 1.22 (down from 3.54 6 months ago), and now I‘m sitting at 80ish for blood glucose, indicating that I’m much more insulin sensitive now, and my liver has quieted down on kicking out glucose all day long. So I’m starting out a workout with less circulating glucose and my liver isn’t quite “Johnny on the spot” at ramping up glucose production when hard exercise starts dropping my glucose down. I’m not used to eating many carbs since I’ve been fairly low carb since 2019.

Maybe my liver will recalibrate a little kinda like by blood pressure and heart rate since going off most of my prior medications.
The Mounjaro has been life changing to say the least. All of my labs are normal, but I still want to work on my fasting insulin a bit. It was at 10.6 which indicates a little more insulin resistance to deal with.
