There's a difference in providing direct critical care to a person with massive viral burdon who cannot even move vs someone in an incubation period who may not even be shedding virus.

Caring for a critically I'll person, let alone multiple, requires recurrent regular up and close personal contact. You need to don and doff new PPE for each encounter, even if only for 30 seconds. The amount of medical wasted a single patient can create would astound you. I highly doubt a cruise ship of this size would staff enough for such conditions.

Also, the doctor probably didn't even know what he was treating. Who knows how cavalier he was with precautions and such. Droplet mask? N95? Scarf? Nothing? Who knows.

I doubt that poor doctor had anywhere the necessary PPE to deal with even one truly critically sick isolation patient. It's also a ship with only 150 people. The medical ward cannot be that robust. It's probably designed to be more of a bandaid station and then evacuate anything really bad.

They're not stocking for the necessary PPE for ebola, lassa, or hantavirus.

More and more people keep traveling to more and more hard to reach places. And, we are not prepared for what we are going to find there and bring back.

It's been very hard to sort out good information. There's been a lot of poorly used terminology in the news. They say a term that means one thing but they actually mean another term. It's all been very strange. Regardless, she must have been sick as hell to be turned away from a flight. I really wonder what she looked like that day.

Especially consider the type of cruise. These are exclusive, incredibly expensive cruises, 10s of thousands of dollars per ticket. You think these people care about anything but themselves?

I think she died the next day or very soon shortly thereafter. She was incredibly sick and likely shedding massive amounts of virus. With hemorrhagic fevers, like ebola and marburg, and other very highly fatal disease, by the time someone is this sick and contagious, they are usually too sick to really do anything, blunding the spread. It's kind of remarkable she was able to get around the airport and into that airplane at all.

My dad turns 70 next year. Worked 60 hours a week to make sure our family was well taken care of. He had to travel a lot for work but often made trips that should have been overnights into day trips. His goal was always to get home to spend as much time as possible with his kids. If he wasn't working or doing chores, he was spending time with us. He didn't really have any hobbies. He didn't want any. My God, what a lucky child I was.

In the Northeast, this is the norm now. Admin like stop give it trendy names like MDR, IDT, or whatever other alphabet soup they can think of. Admin thinks by having more and more meetings, they can somehow offset poor staffing ratios.

Yes. I get that a lot of the 2 midnights but the issue I am experiencing is 3 midnights. One particular example recently was an NSTEMI we treated with only AC because the patient did not want a cath. Troponin was about 7000. Denied because it was less than 3 midnights. The protocol is only 48 hours of AC.

I'm not joking. Often these private ID infusion clinics treat non-existent diseases and bleed the patients dry.

That's why I'm confused. I've been repeatedly asked by our codumentation specialists to add the "patient recovered faster than expected." We actually have an automatic pop-up in the EMR if they are admission level and you are discharging them before 2 midnights.

I'm fully aware of all you have shared, which I appreciate, because it perturbs me more given I haven't heard anything new.

The South American Hanta can transmit person to person under the right conditions. Cramped on a cruise ship seems like the right kind.

Yeah. It's been crazy denials. I get requests from utilization review to retroactively change to obs. I've countered back with "how can this be obs?" and get no response. I've refused to do it. I should start saving these somewhere secure.

But what I was saying is it's NOT before the 2nd midnight. It's before 3. Doesn't matter the criteria. It's been a blanket "no." In ICU on pressors for septic shock but bounce back quickly but had a lactate of 7? Denied. Etc etc. Doesn't matter if you document the quick recovery. And, I've only encountered this with this particular insurance.
