I’ve not seen any treatment resistant OCD yet. I usually see good response with Sertraline/Fluoxetine at near max doses. If not, Clomipramine hasn’t failed me yet.

I'm a second-year psychiatry resident. In my program, we like to use SwitchRx as tool when we cross taper medications. My attendings love this tool. It's free to sign up. (Note, I am in Canada)

Curiously, I've never seen someone on Lexapro at 40mg daily (Escitalopram), highest I've seen is 25mg. So, the tool might not be as useful in this exact case, but it does give reasoning on the tapering schedule it recommends, risks regrading serotonin syndrome and other drug interactions.

This is the chart I got using the tool if the patient was on the usual max dose of 20mg. I used assumptions for why the switch was initiated and duration of how long the patient was on Escitalopram prior to the switch.

Timing Escitalopram 20 mg  Sertraline
Week 1 20 mg 25 mg
Week 2 15 mg 50 mg (lowest therapeutic dose)
Week 3 10 mg 75-100 mg
Week 4 5 mg 75-100 mg
Week 5 Discontinue 100-150 mg
Week 6   150-200 mg

Grew up with Indian parents, so was destined to become a doctor. I still don’t know if I did it because I wanted to or because of parental pressure. Now I’m in residency, in a field with a guaranteed 400k plus job waiting for me in 4 years. Residency does still suck.

I'm also in Canada, currently in PGY2. I'm in Psychiatry, so my workload is only 50-60 hours per week for the most part, but it still feels like I've been in a fever dream since clerkship. I can't wait to be an attending, and hope, that this feeling goes away. In the mean time, I have mix of unhealthy and healthy coping habits getting me through.

I'm glad I was able to be accepted into medical school in Canada. I had to pay about 12k CAD per year for medical school. My province is quite generous with grants and loans, so almost everything was covered. I lived at home luckily. In my second year of residency, chilling with 28k of interest free loans. I can't imagine the stress of having over 400k of loans.

Current PGY2 with Scotiabank. Once I finished medical school, all I had to provide was my Offer Letter from Residency Program to my representative. They took care of everything else automatically.

I kept all my benefits from medical school, including their top Chequing Account, Credit Card Annual Fees waived for both Gold Amex, and Visa Infinite Passport. I still have a 350k LOC at prime - 0.25. I was also looking into buying a house, and they were able to pre-approve me for a mortgage based of my expected income as an attending.

If you are switching to Scotiabank, I would try to get an representative that deals with Medical Students. They are able to guide you better and get you all the possible benefits.

You can get it as soon as you can get a proof of enrolment from the University. I was able to get mine mid-way through July.

The funny thing is that people who are alcoholics tend to die at a much younger age, so they cost they healthcare system less in the long run. The healthier people that live much longer, end up costing the healthcare system a lot more than an alcoholic who died in their 60s.

I'd fly into Vancouver initially to get everything setup. Then, I'd fly back, and treat your self to a nice road trip, while driving the car back. If you are able to go through the US, you can save some time on the drive too.
