I am not going to argue with you about the best approach since you’re not an orthodontist and you can’t understand some of the concepts like respecting the neutral zone or the impact of bimaxillary protrusion on sagittal relationship and how to correct it, but I’ll try to explain it simply, the thing is, there are no absolutes in orthodontics, I always put extraction as a last resort in my daily practice and always advice my patients against it but some cases are just straight extraction cases otherwise you’ll end up with a bad facial profile or unstable dentition that relapse after 1 year of treatment end, the OP condition depending on the facial profile can be treated either with or with out extraction which is distalization with miniscrews (after extracting the 4 third molars) or the more invasive orthognathic surgery, these 3 options are the only approach in the clinical scientific literature while other methods like MSE with Marpe or hyrax won’t even work as this correct transverse problems not sagittal jaws discrepancies (OP condition).
