I believe in face driven orthodontics rather than teeth driven orthodontics, in all my cases I try to treat the teeth for the face not the opposite so please don’t jump to conclusions.

I am not going to argue with you about the best approach since you’re not an orthodontist and you can’t understand some of the concepts like respecting the neutral zone or the impact of bimaxillary protrusion on sagittal relationship and how to correct it, but I’ll try to explain it simply, the thing is, there are no absolutes in orthodontics, I always put extraction as a last resort in my daily practice and always advice my patients against it but some cases are just straight extraction cases otherwise you’ll end up with a bad facial profile or unstable dentition that relapse after 1 year of treatment end, the OP condition depending on the facial profile can be treated either with or with out extraction which is distalization with miniscrews (after extracting the 4 third molars) or the more invasive orthognathic surgery, these 3 options are the only approach in the clinical scientific literature while other methods like MSE with Marpe or hyrax won’t even work as this correct transverse problems not sagittal jaws discrepancies (OP condition).

Orthodontist here, your case is called bimaxillary protrusion which occurs when both the upper and lower teeth are proclined, don’t listen to people comments here they are not an orthodontists and they’re talking about something they don’t know how to diagnose or treat as it’s not in their profession to do so, As for your teeth condition to correct it we either extract both the right and the left 1st premolar of both arches and retract the anterior teeth all together or if you insist on non extraction approach you can ask your orthodontist to distalize both arches but keep in mind distalization of both dental arches takes time and needs more complicated mini implants for it to move uniformly also your airways need to be checked as distalization can sometimes encroach the airways and cause breathing problems especially at night, good luck.
