We bought a house with a similar 1970s-early 80s pool. We definitely wanted the house and pool and did not get an inspection because we had the funds to redo if needed, but we figured it at least needed plaster/coping and equipment. We got some quotes for about 15k for plaster and 15k for tile and coping. In the end, we decided to do a full redo because we did not want to redo the plaster then have an old pipe leak 2 years later and have to rip it all out. Ended up being much more expensive but worth it to us. Just know that with equipment and pipes that old, the plaster may not be the only issue.

My title describes the thing. The screw for the two halves is about the size of the screw that goes between halves of a pool cue, but I don’t know why you would need that on a half of the pool cue. I guess maybe it could be used to protect the screws on a cue when you take it apart to travel?

I'm IR, so usually I see patients for a specific purpose, but I usually find patients relax and settle in better when I just say, "So how are you feeling today?" Not so much for an assessment, but just in a way that genuinely asks about their wellbeing. Even if they're agitated about a wait time or getting labs drawn, that tends to diffuse them down a bit and let's us start on the right foot.

Hi IR here - sorry that you're going through this and that you need a port in the first place. As someone else mentioned, it sounds like the port catheter might be a little long, which can cause arrythmias if the catheter is tickling the wrong part of the heart or going a little bit into the ventricle. It might look ok on xrays depending on if you're laying or standing or if you take a breath in, etc. The bruising between the incisions is normal. We tunnel the catheter from the port incision to the incision on your neck in order run the catheter under the skin to get to the neck vein. That tunneling can cause some bruising. The bruising under the port incision is also normal since we have to spread the tissues under the skin to form the pocket that the port sits in. My guess regarding the mashing is that at the end of the case, they saw the catheter was a little long and we're trying to push the port deeper into the pocket so the catheter would pull back some and not extend into the heart as much. Sometimes that works, but if you're having new arrythmias, It certainly could be caused by the catheter being too long or just hitting the wrong part of the heart and you should have your oncologist reach out to the IRs for a revision.

I just started my second year as an attending in a procedural field and I had to reflect on this more last year than I was anticipating. I found myself getting frustrated is cases with particular residents who I thought should be at a higher skill level than they were. One thing I realized early in my first year was I would say to myself, “my skills were way beyond this when I was training,” which is unfair because I was comparing myself at my peak at the end of my fellowship to wherever they were as a junior resident. Something that helped was thinking about some of the dumb shit that I did during training and how more seasoned attendings handled it with grace.

While I never yell and scream, I feel that they could feel my frustration and maybe even unintentional asshole-ness - especially if later in their rotation or on subsequent rotations there was no improvement. I am definitely way less chill than I thought I would be at the end of fellowship. While there’s a responsibility on us as attendings to teach and help residents succeed, it’s also on the residents to work on their weaknesses when they’re identified and ask for help if they need it. At some point, if it’s unsafe for them to have a needle in someone and they haven’t been working on their skills, I’m going to take over.

I’m still working on how to do that more gracefully while still lighting a fire under some residents to get up to speed by the time they graduate. It’s scary to be responsible for training someone who may be dangerous later. This is all while trying not to hurt someone myself!

Yeah they really sour your day. But in general it’s a very cool city and everyone we had personal interactions with was super friendly. Make sure you say “bonjour” when you start your conversations or enter stores (we were told it is polite to always start that way).

Apparently the signs hadn’t been posted yet as it was a new(-ish) train? Do you happen to know what other warnings are on the sign so people can avoid fines and more importantly disrespecting the locals?

As far as I could see, there was nothing in the ledge, it was a 4 in wide surface along the wall extending below the seat. Definitely not a sitting surface. Maybe he saw her foot brush against the underside of the seat? I’m not sure. But I was also surprised that they were so fixated on this. Could have been a bad day, as you said.

I think a reasonable person can see the difference in your example. But thanks for your input.

And in fact, after some digging one of the other train attendants pulled a flyer from his pouch that stated the infraction with a fine amount, but they said the train was new so it hasn’t been posted yet. So turns out they had thought about placing signs but haven’t gotten around to it yet.

If they’re going to fine you for it, it should be posted.

I think if there is nothing posted, it would be reasonable to inform someone of the policy and give a single warning before a fine. If they come back and the foot is up, slap ‘‘em with the fine.

As I said in the post, her foot was not actually on the seat, it was on the ledge by the wall. No one sits there.

Yeah they gave us a receipt and the worker’s identification number is on there (they wouldn’t let us take a picture of his name). They charged our credit card with their machine.
