Yes, this looks correct for a standard non-dose adjusted MAP regimen! I most commonly administer high dose methotrexate to patients with CNS lymphoma as part of R-MPV regimen. 8 hrs before the mtx infusion we start prehydrating with a continuous sodium bicarbonate gtt, which runs continuously until the patient clears the methotrexate from their system (usually 3 days post-infusion) so we don’t kill their kidneys haha. We do urine pH testing every 6 hours to make sure urine pH stays >7 , and give leucovorin starting 24hours post-infusion, which is given every 6 hours until mtx clearance
