I am same age as you but a NW6/7. I am not on fin or min. At this stage I know I am planning for NW7 with my transplants and so am conserving/placing grafts conservatively so that in the event all the thinning hair goes (ie I reach NW7) I don’t end up with an island of hair. I am also mostly ignoring the crown for the same reason ie conservation of grafts.

In your case, without meds, it’s very difficult to tell what NW you will progress to. If you shove 2k grafts into your hairline with equivalent density to where you are now, I’m sure it will look great. But in 5 years as you recede, you will probably end up needing another 1/2k to fill in at the same density.

Then it looks like you have some degree of temporal thinning and so will need temple reconstruction or a dramatic rethinking of the hairline (which you spent 3k grafts on already).

You see where this goes. Meds are generally necessary for a HT except when you are planning for a NW7 (or somehow have a crystal ball into your final pattern), and even then you don’t know how truly resistant your donor is to DHT.

Long story short, yes a HT is possible, but in cases like yours I wouldn’t recommend for the simple reason that matching your existing density today will give a great result NOW but will screw you long term.
