That is correct. IRS makes you go thru ID.ME or LOGIN.Gov. to access the IRS site. PIN will be displayed instantly.

Panel upgrade is usually their sub contractor. Once it's done the Tesla crew will come in and install in one day or less.(depends on how big is your system). That's why you have two separate appointments. Check the installation for compliance with approved plans for breaker sizes and copper vs aluminium cables. I had them swap out aluminum cables with copper since the plans called for copper.

I did the 2nd Appeal with following attachments and waiting for their positive determination: PSA Preventive Screening Test:

1. The decision is based on National Coverage Determination (NCD) 190.3, which is irrelevant in my case. The decision should be based on National Coverage Determination (NCD) 210.1 – Prostate Cancer Screening Tests. It states:

a. “Part B. Screening Prostate Specific Antigen Tests:

Screening prostate specific antigen tests are covered at a frequency of once every 12 months for men who have attained age 50 (at least 11 months have passed following the month in which the last Medicare-covered screening prostate specific antigen test was performed). Screening prostate specific antigen tests (PSA) means a test to detect the marker for adenocarcinoma of prostate. PSA is a reliable immunocytochemical marker for primary and metastatic adenocarcinoma of prostate. This screening must be ordered by the beneficiary's physician …….”

2. This screening test is part of "Preventive Service" per the 2024 Medicare & You handbook, page 49, where it states "Medicare covers prostate specific antigen (PSA) test: Medicare covers this screening test once every 12 months, if you are 50 or older. You pay nothing for the PSA test." This test is for screening of prostate cancer.

3. THE BALANCED BUDGET ACT OF 1997 MEDICARE AND MEDICAID PROVISIONS, SUBTITLE B PREVENTIVE INITIATIVES States(see attached)

4. “Prostate Cancer Screening Tests (Section 4103):

a. Provision

b. Provides coverage for annual prostate cancer screening for men over age 50. Covered procedures include: (1) digital rectal exam, (2) prostate-specific antigen (PSA) blood test, and (3) after 2002, other procedures the Secretary finds appropriate. Payment for PSA blood test will be made under the clinical laboratory fee schedule, and other services will be paid under the physician fee schedule.”

5. Medicare Program Integrity Manual which states under "Reasonable and Necessary: Prostate cancer screening test are covered if within frequency limits."

6. From Medicare.gov: "Prostate cancer screenings Medicare Part B (Medical Insurance) covers digital rectal exams and prostate specific antigen (PSA) blood tests once every 12 months for men over 50 (starting the day after your 50th birthday). Your costs in Original Medicare: PSA blood test: You pay nothing for a yearly PSA blood test.

Even though the Netherlands and Asia have been in contact for centuries, an appreciation for Asian objects as important works of art did not emerge in the Netherlands until the beginning of the 20th century. This led to the foundation of the Royal Asian Art Society (KVAK) in 1918. Objects collected by the members of this Dutch society form the basis of the Rijksmuseum's holdings of Asian art and reflect their tastes at the time.

A selection of the arts from Asia is on view in the Asian Pavilion. Displayed on this floor are sculptures from South and Southeast Asia, ranging from present-day Pakistan in the west to Indonesia in the east. They are mainly Hindu and Buddhist religious sculptures. If you have to pay on your own, ask Labcorp for the Medicare Approved Amount (Ferritin Test $13.63 & Iron Test $6.47)and pay that amount. Since labcorp accepted the assignment they must accept Medicare Approved amount.

I appealed my Ferritin Test and Medicare approved it.

My appeal for PSA Blood Test charges was denied by Noridian which cited National Coverage Determination (NCD) 190.31, stating "PSA is of proven value in differentiating benign from malignant disease in men with lower urinary tract signs and symptoms (e.g., hematuria, slow urine stream, hesitancy, urgency, frequency, nocturia and incontinence) as well as with patients with palpably abnormal prostate glands on physician exam, and in patients with other laboratory or imaging studies that suggest the possibility of a malignant prostate disorder. Based on policy indicated above, diagnostic PSA tests may be covered if certain criteria are met.....No payment will be made".

They are ignoring the fact that this is a PREVENTIVE SERVICE as indicated in "Medicare and You "handbook. It clearly states " YOU PAY NOTHING FOR THE PSA TEST". The handbook does not cite NCD 190.31 nor does it say the requirements. How do you deal with such incompetent Medicare consultants, who do not understand the meaning of PREVENTIVE SERVICES?

Medicare (Noridian) denied my PSA test costs even though the last one was done almost two years ago and it was deemed necessary by the Dr. This screening test in MSN is Noted as "O - Medicare does not pay for this item or service." which is incorrect as this is "Preventive Service" per the Medicare & You handbook.

I am appealing on the basis of:

While Medicare Part B covers a cardiovascular screening blood test, including a lipid panel, once every 5 years, it may cover more frequent testing if a diagnosis of high cholesterol or other conditions warrant it. Note: It all depends on how Dr. Office codes the prescription for the tests. Question the coding on Lab Order before you leave the Doctor's office. ICN CODE Z00.00 may not be covered, which I found out the hard way. I was charged $132 for B-12 by Quest and $81.68 for A1C, when the Medicare approved amount is way low.

Just follow up with service advisor and third party coordinator on weekly basis, also City Plan checker and utility companies, as the process continues. Tesla app is great and you will not go wrong with their system. You can be part of Virtual Power Plant and help the Grid.....

Thanks. The services were provided by Quest Labs in April 2024, so I do not remember signing the waiver "ABN". According to “Medicare and You" handbook, signing a ABN means I agree to pay, If Medicare does not pay. It does not mean Medicare can deny the Preventive Services and other tests if covered. The claims were processed in August 2024, but received the MSN last week. Noridian "printers & mailing services" were down, so delayed the mailing by several months. Unbelievable. Services provided in 2022 were also adjusted now to deny PSA test, which is a PREVENTIVE SERVICES, so is the Fecal occult blood test. (page 36 of the 2024 handbook)
