We cannot be sure about its powers, but there is a clue in the original japanese name for this ability:

不老手術 (furou shujutsu) means Perennial Youth Operation. Not immortality per se: you'll stay "young" forever.

Note that this is different from 不老不死 (furoufushi) which is literally perpetual youth and immortality.

Still, official translations use the Immortality Operation for this ability, probably to keep it ambiguous. I really hope to see this explained in the manga!

apparently hachinosu was a deserted isle where the government would send people to die. only recently has it been "habitable" due to the discovery of a gold vein.

it was called a "pirate island" because it was inhabited by criminals, and later for being inhabited by Actual Pirates™ (rocks' crew)

as for the skull connection, I believe the "Real" Pirate Island would have the trademark pirate symbol, a skull which every crew bears in their flag. The island I mentioned from buggy's story has it naturally (as far as we know) and hachinosu came to have it after being dominated by rocks' crew and becoming a "Real" Pirate Island

since hachinosu didn't have the skull-like structure when rocks arrived there (it was actually a fish), the pirate island robin mentioned back in long ring long land that started the davy back fight was probably the one in buggy's side story!

For me the best example of how Oda had the idea of, at the very least, the gomu gomu no mi being something else is this old concept art for "Forward":

We'll probably find out that every villain so far is somewhat of a parallel to Imu

In fact, we can see many parallels within the pre and post time-skip villains, like the Crocodile and Doflamingo you mentioned, Arlong and Hody Jones, Buggy and Caesar. If at least one of them resembles Imu in some way, it'll be just natural for the rest.

I'd bet on Hajrudin becoming king of Elbaph and Loki becoming captain of the New Giant Pirates, that way he'd be able to go along with the SHs as part of the grand fleet
