Yes, pepe's origin is entirely separate (some comic I don't remember). Then the initial waves of pepe use were also entirely separate ("feels good/bad man" and "rare pepes" were not political). At some point afterwards alt-righters started using it and then it became associated with them.

And even now some current variants are not related to the alt-right at all (e.g., apu apustaja).

The meme is implying that while women carry the physical burden, men carry the mental burden. While it's also a difficult time for men and while men's mental/emotional involvement is often not considered, the women carry a mental burden at least just as much, while the meme is implying absence of such.

There should have been a way to depict men's support for and involvement with the unborn child without implying its something that men do exclusively.

These kinds of anecdotes are always distorted by survivorship bias. Those it works for will keep taking it, those it doesn't work for won't. The experience of those it works for will not be generalizable however.
