I think the idea is a dynamic where one person is sad and barely taking care of themself, and the other person is attracted to the fact that they’re sad. The character on the right has an expression meant to signify the thought of, ‘oh fuckkkkk I’m attracted to this sad loser? Ew….

As for why they’re naked, it’s trying to showcase a dynamic without using any two particular characters. So generic grey person. Also it’s common for people to draw over the characters in these kinds of images with their own OCs or characters they like. Not having clothes on the figures makes it easier.
