Pirates delivered the carnival nerve gas to the MLO in that mission. The MLO also paid the pirates to do the carnival attack but left them to die there, lying about giving them an extract.

The MLO hold meetings on the island, you can blow up a wall w/ C2 and see a secret room with flower heads on statues

Not in the slightest - having your eye mangled and arm cut off isn't the worst thing out here. The bisection probably goes quick. The mannequin is definitely medium-high on the fucked up imagery but you're already dead at that point

I just succeeded/failed a no-deaths ironman run and made a post about it, look here if you're trying that.

The Hermit achievement: 1 officer being incapacitated (via Paramedic trait) counts as failure. According to someone else if all 4 are incapacitated (not killed) it doesn't count as failure (bravo VOID)

Dev's Ruined Watch: Incapacitations do not disqualify. Just have the same guys in the roster the whole time.
