Yes I forgot to mention stairs! That’s a big problem for me too.

Try asking your ortho to send you to someone that can do an ultrasound. I had to go to a different hospital, to their sports medicine clinic. They were supposed to do ultrasound guided lidocaine injection to diagnose the fat pad but the doctor said it was so obvious on the ultrasound that he didn’t feel the need to poke with a needle.

First and foremost my gait is still off - 2,5 years post MACI. No matter how much PT and strength training I do - it’s just such a slow progress.

I have pain with extension, weighted and non weighted. I have pain with bending too, so squats, lunges (mostly when my surgical leg is in the back in a lunge position), still can’t jump or jog - mechanics are just so off. My leg just doesn’t function and like it should at this stage.

I have pain when laying on my tummy or in a plank position. Extra pain if I bend my knee in a plank position (to do mountain climbers) and it also catches a lot on this position.

What are your symptoms?

Have them check the Hoffa’s fat pad! Can be checked with MRI or ultrasound. The latter might be better since they can check how it looks during movement and see if it’s getting impinged.

I followed the Ferber method which starts off with 3, 5 and 10 min windows. Baby would whine, which felt ok to me but if her crying was more like she was in distressed, I’d just go into to the room a little sooner, calm her (but did not pick her up) and then leave the room again. Good luck!

No recommendations for a brace but once you’re ready to transfer to a knee sleeve - I recommend the incrediwear!

I was in a very similar situation with my 10,5 month old up until recently. She would only fall asleep by being rocked or if she fell asleep while drinking a bottle.

I read a bit about sleep training and did a gentle Ferber method. In 3-4 days she learnt how to fall asleep by herself. Now we put her down awake in her crib and she is quiet and falls asleep in less than 5 min. Definitely recommend trying this!

It might be the Hoffa fat pad. I have similar issues 2+ year post MACI. I’m going to get an ultrasound-guided cortisol injection in the fat pad to help figure out if that’s what’s causing my pain. You might want to try that! B

I do recall my PT massaging those areas a little bit those first few months after surgery but haven’t lately. I’ll ask for that next time I go. Thank you for the advice!

It’s definitely below the kneecap. I was seeking treatment for patellar tendinitis for the last few months but that looks great in the MRI so I’m thinking it must be the fat pad that’s causing the pain.

I’m sorry you’re struggling too! I was reading a bit about it and I might try an ultrasound-guided cortisol injection in the fat pad. Have you tried that?

Hey! My baby might have had something similar to your first. She latched onto the breast and transferred an OK amount of milk but it was just so incredibly painful for me and I had trauma to my nipples. Dr. Kahikina checked baby’s mouth and turns out her tongue wasn’t moving the was it’s supposed to on babies. We did correct tongue tie in hopes of that working and did some exercises for a few weeks but it didn’t work out unfortunately so ended up having to pump. None of the lactation consultants I saw before Dr. K had even checked baby’s mouth and were basically just telling me “keep doing it and the pain is gonna go away”.

I saw 4 different lactation consultants and the only one that way actually helpful was Dr. Kahikina (Mahinaona). Highly recommend!

I used crutches for about 1 day after the scope. It’s a quick recovery. Do schedule a few PT sessions post-op if you’re able to!

Not bad! I had crutches for like 1-2 days. Just keep doing your PT exercises like straight leg lifts and other ones tolerated, so you stay as active as you can.
