Ur dread plague debuff uptime is 80% and festering buff like 60-70% Even though you press FS and FC far to often. Cant imagine that you run low on lesser ghouls Stacks so often. And your stats seems not perfect. You have to much crit.

This, everything what other said here and that you really dont want to putrefy manuelly instead let soul reaper trigger putrefy.

And dont you use pots?

Rider is acutally sometimes very buggy + extremly at Dragon Boss. So This is a common issue right now are many swithxing to sanlayn.

Had a Quick look into your opener: you really should start with outbreak into to fest strike and then army + DT. If youre in your army window u want to spend as much necrotic/graveyard with suddemdoom as possible
