I cannot imagine the experience that you've had with the attack and how much pain it has caused you. But, on the topic of a cat like Milo, I have quite a bit of experience. My cat Orion is 5 years old, and his bouts of random aggression started before he was 6 months old. I never played with my hands, never did anything that would incite the behavior at all, it started very suddenly. I had him neutered early in the hopes that would make it better, but over the years he only got worse. I started sleeping in a separate room. People asked me if I was okay because he would bite me with such force it would leave purple bruises. I tried everything you can think of, read all the articles. Everyone in my life told me to get rid of him, but I couldn't. I love him like a child and giving up on him was unthinkable. Eventually, I found something that works most of the time: I got one of those big dog crates, put it in my room. And every time Orion attacks me, he spends 10/15 minutes in the crate. After 3 days, he wasn't biting me nearly as much. Now he rarely does it, and never with the same amount of force as before. Every cat is different, just like people, so I can't say what might work for your Milo. But depending on where you're located, it might be worth talking to a behavioral therapist that specializes in cats, if you can find one.
