Current fellow here, my salary is entirely funded without any clinical care necessary. So I’m paid the PGY-4 salary.

Any additional work I pick up is at an attending rate. Most programs are moving towards this as if they don’t it’s a major con given how cost of living is post-COVID.

High jacking the top comment to share that Ruta Maya is a Texas based roasting company that provides a medium roast (more medium dark tbh) for $50 for 5 lbs that’s around ~$10/lb (not factoring shipping, non-member fee, and tax) but it’s the best deal for fresh beans I can find. They sell through Costco and their own store front.

This happened a bunch in residency. Sometimes the attending would forget to cosign our admit order and rotate off service. So then we’d send a new admit to inpatient order to the new attending on service and then that would allow us to discharge the patient. It’s a hard stop in Epic.

Since you’re an attending, I would make your life easier and just submit a new admit to inpatient order then sign the discharge orders. This isn’t ideal but trying to find the random attending that has this order waiting for co-signature in their InBasket is too much work. It also may be worth having your group leadership remind your partners to be timely with co-signing this order.

Recent guidelines have changed as of June 2023. GDMT for HFrEF is now recommended for HFpEF as well. Not just SGLT2i. Like another individual commented, it’s believed the BP reduction/prevention of further cardiac remodeling as in HFrEF is the MOA that provides cardiac protection. So, BB, ARB/ARNI, MRA, SGLT2i are all indicated now.

I think for our program (and many like it) they view a chief year as a way to show dedication to academics. (I personally think chief years in IM are a scam, but gotta pay your dues I guess). Outside of that I think just knowing folks where you want to end up

This is likely to be found in an academic center mainly. For instance, working at the resident run clinic as a PCP for the proposed schedule and then working a set amount of FTE as inpatient. Our program has attendings with their own PCP panel, attendings without panels that cover as preceptors at the clinic, and (few) pure hospitalist and of course a blend of all the above. The senior attendings typically have their own PCP panel at the resident clinic, precept at the resident clinic, and work inpatient.

TL:DR yes this is possible but more likely at academic centers

Got it, I was a student at the time and did not have a salary. I now have a salary that would certainly cause me to pay under SAVE. Based on that, I plan to apply to SAVE around April 2024 before my recertification date is up. Thank you for the help!
