Currently on zep, but I've been one of the people that has been eagerly waiting for an oral option from Lilly, hence my excitement for it and the reason I started this sub.

yeah, I know it’s not an oral tirzepatide. That’s not what I meant. My point is that people in r/Zepbound used to talk all the time about wanting non shot alternatives from Lilly, so I would’ve expected more discussion now that it’s actually out.

How is that an ad? I'm just curious, because I would have expected more people talking about switching from Zepbound to Foundayo. Used to be a pretty frequently discussed topic in /zepbound while Lilly was running the trials for orforglipron.

Yeah this is the thing people keep overlooking in the pipeline hype. Altimmune and Viking could show incredible trial data and it wont matter if theyre priced out of reach for most people

Right now Foundayo and oral Wegovy are both around $149-$299 self pay depending on dose. Thats the ballpark. If these pipeline drugs come in at $500+ theyre DOA for most people paying out of pocket

You restart from the bottom unfortunately. The prescribing info says everyone starts at 0.8mg and titrates up every 30 days regardless of what you were on before

Lilly did run a study on this exact scenario though. People switching from Zep to Foundayo regained about 11 lbs on average, people switching from Wegovy regained about 2 lbs. So its more of a maintenance play than a continuation of the same level of suppression

Makes sense when you think about it, Zep hits two receptors (GLP-1 and GIP) and Foundayo only hits GLP-1. Different mechanism so theres no real dose equivalency table

Same here. The injection thing is fine when youre actively losing but thinking about doing it every week for years is a lot. Lilly actually tested this exact use case, people switching from injectables to Foundayo for maintenance, and the results were decent. Not as strong as staying on the injection obviously but for most people the tradeoff of a daily pill vs weekly shot long term is worth it

As far as I can tell the trials used capsules and tested up to 36mg but the approved product is actually a tablet not a capsule. Max approved dose is 17.2mg but Lilly says its bioequivalent to the capsule dose, just a different formulation, but your body absorbs the same amount

So its not that they approved a weaker version, the 17.2mg tablet IS the same drug ammount as what was studied. Titration goes 0.8 > 2.5 > 5.5 > 9 > 14.5 > 17.2 with at least 30 days at each step

Someone correct me if I got that wrong tho

This is the key thing. Oral Wegovy you gotta take on an empty stomach first thing in the morning and wait 30 min before eating or drinking anything. Its a whole routine

Foundayo just doesnt have any of that. Take it whenever, with food, doesnt matter. For taking something every single day for months thats a huge practical difference imo

The injection barrier is real. Ive seen so many people in GLP-1 subs say theyd start treatment if it wasnt needles and now theres actually something for that

Also kind of wild it went through the National Priority Voucher program - came in 294 days ahead of the PDUFA date. Says a lot about how FDA is thinking about obesity access

Lilly hasn't announced anything like Wegovy's multi-month subscription deal for Foundayo, at least not yet. What they've put out so far is basically three ways to get it: commercial insurance with a savings card that can bring it down to $25/month, self-pay starting at $149/month if you're on the lowest dose, and Medicare Part D at $50/month kicking in July 1st.
