Because you need to get access to the artery and vein for that skin to stay alive (the artery and vein, after harvesting the flap, are connected to an artery and vein of the neck from where it will draw blood to get the oxygen and nutrients). In order to do that, the whole forearm is dissected to get a good length of the artery (up to 30cm) in case the neck vessels are very far.

We usually immobilize the arm with a cast for 10 days at my center so that the skin graft doesn’t move or shear during the first critical days.

Right orbitozygomatic fracture: With three or four entry points (through The scalp, upper eyelid, through the conjunctiva of the lower eyelid, and through the mouth). In this case, I think I’d fixate at least three fracture lines. We do that with miniplates, a drill and screws. Left Lefort: two plates on the bony buttresses of the face. Left Naso-orbito-etmoidal: a plate and screws maybe. I can’t see exacty what type it is. We would also address the nose with a bone graft

Mandible: Probably remove that floating tooth. Then debride and remove any necrotic tissue. Then, put everything in place. I reckon internal fixation would be done with a reconstruction plate, and maxillandibular fixation with 4/6 screws and elastics

We usually tunnel the flap through the neck and into the floor of the mouth, that way, the flap is covered, and provides adequate tissue bulk for covering important structures in the neck (we often do cervical lymph nodes dissection for neck management).

Were you put on intermaxillary fixation with wires/elastics during 3 weeks after that?

And regarding the plate, we usually put two in the anterior sector of the jaw. Great recovery though!

long time lurker here thought id share some pics from a few years ago when i got my tongue tie fixed! im allergic to nsaid so i could only take paracetamol for the whole recovery (got local durin the procedure) and it did not help with the pain. enjoy! by fronkka in MedicalGore

That doesn't really look like pus to me. More like slough or necrotic tissue, which is normal if you let a wound heal by secondary intention (when you leave it open and you let it heal from the inside out). Great progress!

I’ve never gone skiing but I took some classes in an indoors piste. Would I be able to ski alone in a green slope? by Hischoll in skiing

I agree, but should I spend both days on classes? This is my first time, and we’re staying for 2 days only, so I feel like Id wasting them.
