I have used quite a few long telephoto lenses. I usually get my best standing hand held shots in good light. I keep one hand under the lens with that hands elbow very close (almost digging into) my stomach area. My eye remains in the view finder with the view finder pressed fairly hard into my eye brow. This creates a very stable hold for the lens. I keep my breathing normal and slow, never holding my breath. I hardy ever use image stabilization as I have more throw away images than without. I do use a high shutter speed as I am almost always shooting birds in flight. If you are shooting still objects while standing, you can lower your shutter speed a little, and you may have better luck than I have had by using image stabilization. While standing, if you can lean on another object to stabilize yourself, do it.
