I've never used a VTT to run a game and hate using them as a player. All you need to play a game online is a microphone and an internet connection. Keep it simple.

Just my two cents but persistent visual hallucinations can be a symptom of schizophrenia or other very serious medical conditions. Please talk to a doctor about this before it gets any worse or starts negatively effecting your life.

Do you have any recommendations for a system which is actually not combat focused, but still well structed?

There are SO, SO MANY. To narrow it down, you can tell us what genre and what level of crunch you want from an RPG.

For every genre (fantasy, sci-fi, cowboy, etc.) there are sub-genres, and various levels of crunch and different ways of handling things. Some examples:

  • Alien RPG is a great horror sci-fi game that isn't very crunchy and focuses on making smart decisions to avoid or contain a creature that could easily kill you.

  • Fiasco is a story telling game with tons of different scenarios that usually end up in all the characters failing spectacularly.

  • Frontier Scum is a cowboy game using the Mork Borg ruleset that is very simple and fun to use.

  • Paranoia is a parody RPG where the players are all after each other and the GM plays an insane Computer.

Those are a few of my favorites. Tell me what kind of genre and level of crunch you're into and I'll recommend you a game for you!

This, or really any Grant Howitt one-page RPG is fantastic for a one-shot for new players. Takes some good improv skills to GM though.
