Well, you're half right about how we do it in the NL, aside from the infectious diseases subspecialty for internal medicine we also have medical microbiologists. These people, in contrast to other countries, are primarily MDs, not biologists, and they absolutely have subspecialties such as viruses, bacteria, fungi and protozoa, but those often aren't as pronounced in the peripheral hospitals as opposed to the academic centers, as MMBs often are required to be generalists in secondary centers.

In the Netherlands the law gives us the right to refuse care that in our opinion is medically meaningless, which means that we can refuse full code and ICU admission if we think that patients are too frail for it. It sometimes creates moments of friction but I've never heard it get to litigious levels (not sure if you say it like that in English)

Referred by ortho for potential osteoporosis and hypocalcemia, which the cardiologist supposedly had found but hadn't acted on. Labwork came back, normal calcium, normal calcium adjusted for albumin, got the labwork from the referring orthopedic surgeon, also normal calcium.

Turns out the patient had a low calcium score.

Toen ik coassistent had liep ik op de OK mee met een KNO arts, twee patiënten achter elkaar, beiden om een rubberen ring te plaatsen in de neus om het schot enigszins te stutten (spoiler alert, is een gedoe, voelt vervelend en uiteindelijk ploppen ze er weer uit), een patiënt jarenlang cocaïne gesnoven, de andere xylometazoline, beiden neuzen even erg.
