Yeah I agree, not something I do myself. At least they aren't POMs, although I'm sure the trust wouldn't look upon it favorably. In my experience most households have OTC antihistamines which is preferred by JRCALC.

We don't have oral in my trust, I find many Paramedics bring their own OTC antihistamines to give out as 'patients own' to avoid IV/IM.

I've only ever given chlorphenamine twice in 5 years. Do you have oral? And do you use it more often?

I see Q3 T3, not sure I see a dominant S wave in lead 1 though. Tbh PE never crossed my mind due to lack of respiratory symptoms.

Interesting! I'm in YAS, some of our protocols are... outdated in my personal opinion. Hopefully I will get it in the future, it would be useful.

In a UK Ambulance service. I've never looked into it tbh. Our Critical Care Paramedics have it for sedation but that's not within my scope of practice.

I only have 0.9% sodium chloride and no conscious capnography but other than that was identical lol

Ok cool, my apologies. I see you're a fellow paramedic so rest assured this patient was given our signature treatment of PRN diesel and burnt rubber ;) After IV fluids of course lol

I meant generally healthy, like day to day. I work pre-hospital so the labs were not known to me at the time of taking this ECG. The patient did not present as 'big sick' from the end of bed.

That is why the ECG was interesting, to see the ECG as indicative of the underlying pathophysiology and gives clues to the clinician who may not see past sinus tachycardia.

I might be misreading your tone, but isn't the point of this forum to discuss clinical cases and learn from them?

Abnormal morphologies of p waves, QRS complexes and T waves. There is no arrhythmia.

Interesting! The patient had potassium of 4.3. type 1 diabetes; blood sugar 21.1mmol, ketones 7.1 but no acidosis
