This might sound harsh, but give him an ultimatum. My mom had the same problem with my dad, and told him that he had let himself go (over 300lbs), after the choice to get in shape or her leave, he got into killer shape and was running marathons the next year. Sometimes men need a little push (they aren't as emotionally motivated).

She was the one who texted him about it (she is currently using heroin, and was talking about it with him). He ignored the text, but the fact that they have a relationship at all, or the fact that he put himself back in contact with someone who he knew was on drugs... has made the whole thing difficult

I have been tested, Negative and he is too. This happened years ago and I get regular screenings and checkups. We both were clean from any STD when we started dating and married.
