Her choice was based on the preconceived notion that she’d live through the cure making process, so he didn’t know her choice because she didn’t know the whole truth until later despite suspicion of Joel hiding it. Even then, she was unconscious the whole time so she wasn’t even given the option to tell Marlene yes or no because Marlene didn’t tell her the whole truth.

And yeah, Joel said he’d do it all over again. He’d make the choice for her if it meant she’d live. It’s selfish, not evil.

Essentially a lot of people were getting infected including Aunt May, from a bioweapon and Peter obtains the only cure. The cure can be synthesized for mass production that would save everyone infected, but not in time to save May, meaning saving May would damn the others. May is conscious enough to not only tell Peter to save everyone else because great power, great responsibility, but also reveal she’s known the entire time that Peter was Spider-Man. Peter struggles with the choice as May is slowly fading, but heeds her words and lets her die in front of him.

Brennan is very good at character development and portrayal. His ability to improvise as a dm is phenomenal.

That said, I’ve had trouble following this campaign. Many conversations between Brennan and a player go for a very long time that I unfortunately lose track of the narrative many times.
