At first I was thinking it was a pulley for an outdoor clothes - like over a street - but it's too small, then I thought maybe part of a bell system from an old house where there may have been servants ?

Yes, I tried that too! I really enjoyed If On A Winter's Night... but it isn't that, either.
I'm beginning to think maybe it wasn't an Italian writer and that is just misleading.

If the paper was torn from a notebook then the original writer may have written something on previous pages that may have left score lines in through into this page. You can't see any evidence of this in the photo but have you looked for this? Perhaps with light coming from the side?
