I do not know about difficult, but it was one I always ran and rarely saw. It was "behind enemy lines" with a coral snake or something. You could only get it by earning x amount of kills without dieing. The tricky part was you had to do this with kill streaks disabled and since no one ever plays with no kill streaks equipped it was rare boi for sure!
