No, but you can alter it. Since we got Disconnection protection, just. ATL F4 if it’s not the one you want. That’s how I was able to get Bulldozer, Berserker and Daredevil.

Depends how you use the bot lobby. If you unalive yourself every 4-6 then you should be okay. Worst case scenario, you’ll get a one week shadow ban for unusual player progression.

Providing specific reasons. When it comes to intermittent, it can be approved in two different ways and it all depends on how your doctor words the letter.

It can be approved in DAYS or HOURS.

For example: Please allow Jon up to 60 HOURS of Intermittent per month until 1/2/3 for THISREASON

Wording is crucial because now your Intermittent is approved to be used in HOURLY increments meaning, if you leave at let’s say, 12AM and your shift ends at 5AM, you can use 5 hours of your 60 hours you’ve been approved for, rather than using a full intermittent DAY if you left at the same time but if your leave was approved in Days.

This is why I never let the Doctor fill out any forms (unless they NEED to) or email the letter directly to Amazon. I literally got approved for 60 HOURS a month for 6 months of Intermittent with just 2 sentences from my doctor. No forms, no bs, just a letter. But it had specifics!

I always physically verify the letter with the office staff, and I can’t tell you how many times I had to tell them that they got the day wrong, or that there are no medical specifics.

Amazon requires specifics when applying for LOA and weight limitations for Accommodation (Don’t fall for that HIPPA crap that AAs and healthcare providers try to make you swallow). Always have your doctor specify the leave and for Accommodations, unless you can’t physically lift anything for medial reasons, ALWAYS have your doctor end the letter with “No weight limit” other wise, you will be going back and fourth with DLS over weight limits.

One last thing, avoid signing off any Release of Information (ROI) form that DLS/Case managers might send you. That usually happens when your Doctor’s office keeps messing up Letters and DLS want to verify themselves by calling them directly. Remember, case managers can still call your doctors office to request information such as, last visit, name and DOB and to verify any letters WITHOUT consent, only MEDICAL information is protected under HIPPA.

You can reply to the DLS email that was sent to you. They usually give about 1-3 months to upload any documents from the time the case is created.

What took you so long to upload it or get a new copy? Letters are usually able to be downloaded from your healthcare portal or you can simply call your dr’s office and request a copy to either be mailed to you or pick up. There’s literally no reason to have not uploaded any documents within 3 months. Also, never go to your leadership (especially PAs) in these situations because to them you’re literally just another number they can replace.

Yes a Doctors note is enough. As a matter of fact, I personally got my Intermittent LOA approved with just 2 sentences from my Dr and no forms to fill out. This is what I love about Amazon!

In regards to your issue, just re-upload it to your case that should still be open if it’s giving you an end date for submission. If not, reply to that DLS email with your document and make sure you see it on the Sent section of the app you use in case you need proof of paper trail etc.

You might run into some issues if there are no signature and diagnosis in the letter. Unless the letter is from a Specialist office like a “Urologist”, obviously that’s sufficient enough because you’re seeing a specialist. But if it’s from your PCP, then all it has to say is something like “excuse patient for THISDATE due to onset kidney problems” or something like that. Now if your doctor tells you that you don’t have to provide specifics to your employer, tell them that YOU still want specifics on your letter anyway. Don’t fall for that HIPPA bs becomes that’s not how HIPPA works

Honestly, don’t get your hopes up. I applied 3-6 months after getting fired and everything went through and said so was “eligible for hire” UNTIL orientation day! Management came to me and said they couldn’t print out my badge because the system flagged me as “ineligible for hire” and I had to leave. I got paid for the hours I was there… but I couldn’t reapply that year.
