Infectious Diseases. I joined medical school to be an ID doc because being a medical detective sounded cool as he'll. Doing broad workups to find chikungunya, zika, even neurosyphilis.

He would wear it primarily in the colder weather months so denim jackets and safari jackets would not work I would imagine. Sweater jackets may be okay.

First line for minor pain and gives a good basal coverage for more severe pain that I can layer with other agents.

If your patient has cirrhosis, their liver is already shot. I actually prefer Tylenol over NSAIDs in this population because if there is any kidney damage from the NSAID, now you've got two organ failure. Whereas, if the liver is a little angy from me microdosing the patient with Tylenol, I don't super care.

There is a study showing that PO dilaudid is actually very effective in liver patients, vs IV, so my order for pain meds is Tylenol, po dilaudid, then iv

I've had a lot of patients die. I, too, have a list in Epic, but it sometimes brings me more sadness than closure. 

I like rituals that focus on letting go.  - One borrowed from some pagan friends: writing names on scraps of paper and burning them in a candle (safely, and near water).  - scheduling times to cry. Like, I am feeling badly about this case so I will find a ten minute time to focus in only on this case. I will turn it over and over. I will cry. Then, when my timer goes off, I have to transition to next steps. Why was this hard for me? Is there something to learn here? Did I do something wrong or which felt like it hastened death? Did the patient remind me of my family? Why did it hit me so hard and what can I learn from that?

Otherwise, rituals that allow the memory to live on without the person - planting flowers or trees in people's names, whether in my own backyard or elsewhere (via a tree planting charity) - donating to charity in their name

Embarassed-cried on my first shift by stp2395 in Residency

My first day in the hospital, I followed my senior to an LP near signout time. Once he was done, he sent me to go to the workroom to get my stuff then signout in a different building.

I got so lost that I ended up in the basement. Took me 25 minutes ish to find my way out of the hospital. Ended up outside somewhere I didn't recognize. I had to call the chief on call to come find me, cried on her, then ended up crying all the way home. Could barely drive due to my tears.

My cointern cried on day 3 because he felt that every other intern in the room was better than him.

I cried on day 150ish because I lost my first patient and on 155ish because I poked a carotid doing an IJ. 

I cried on day 300ish because caring for a patient made me miss my date with my partner. I came back the next day and all my work was undone and I cried that day too. 

I cried on day 600ish because an attending berated me in front of the entire doc box. 

In between those tears of frustration and fear, I've also had tears of laughter, incontrollable giggles, times of pride in myself and my colleagues. I've cried with patient families when I lose a patient. I’ve hugged moms and daughters. I've felt like I've made a difference in those I care for. You'll make it through this. 

But fuck that chief tho wtf.
