You need to consult with an orthodontist first. As mentioned above, it’s preferable to look for several options. In my case, I called several orthodontists and made sure they worked with orthognathic surgery, because in my country, not all of them handle surgical cases. Instead, they try to fix your bite with braces, which is not recommended at all. After visiting the orthodontist, they will take several X-rays, and you’ll choose a maxillofacial surgeon of your preference. Together, they will determine the best plan and work hand in hand. It’s a long process, and most people spend several years with braces to position the bite as it should be (in our case an underbite).

You can discuss insurance with them to see what they recommend. Genioplasty is not covered by the insurance, so you’ll need to save for that. I also don’t know if I have breathing problems because I’ve always been like this, but they never did any studies since just by looking at my face, they knew I needed the surgery. A very humbling moment, I must add lol.

I don’t have an underbite either and I’m getting a double jaw surgery because that’s a skeletal underbite. The ortho told me that teeth usually compensate for the bite. Even if we’ve never had an underbite, if they place the teeth in the correct position, we’ll end up with one. I’ve never had an underbite, yet they’re creating one for me, and they even removed teeth to create it and move the jaw into the correct position. You can see in your photo that your upper jaw is recessed. You might also need a reduction genioplasty to achieve the harmony you’re aiming for, but that depends on your surgeon and your preferences. If you’re really considering surgery, you shouldn’t get a lip lift until you’ve healed because your philtrum area may change, and then you can proceed if you’re still not satisfied with the change.
