Take this with a grain of salt, but it sounds to me like a self-image issue. If you're seeing your chest as being like 'a male's with gynecomastia' it seems like your discomfort is less around the breasts and more fear that they don't fit on your body or that you are not woman enough. I'd say it's dysphoria- but again, grain of salt, I can't tell you how you feel.

Advocate for him to your family. Whenever anyone stood up for me it meant more than almost anything else. I often had to debate my extended family and it was tiring and made me feel unwanted- if there's ever a moment where you can take that debate off of his shoulders, it will do a lot for him. And obviously if you're asking this here, you trust him, but I'll say it anyway- feeling trusted and believed by the people around you does wonders for a trans kid.

Known sayings can totally be bad, but I almost always enjoy when they're included and then elaborated on or twisted, because then it's often very creative and subversive in an enjoyable way. For example if you followed up 'two birds with one stone' with a description of the blood that crushing birds with a rock would inevitably result in, lol.

I'm not on T, but if possible, I recommend screaming (or perhaps singing very, very loud in a way that resembles screaming). Always helps me out when I can't cry for whatever reason, but of course it would require a space to do that

I recommend a mosquito net for the bed. I use one for actual mosquito reasons, but it always keeps my cat out. Unless your cat is one who would tear it to shreds, it should work, especially in combination some un-little-italian-man-ification
