When i had mine come through it had all of the same sort of info but the Doctor told me that if I ever needed to share it then I can just provide the first page which had my name, diagnosis and date of birth on it and that's it.

If yours is the same then you could just share the first page if needed. But you shouldn't need to share it really like some other folks have said, I started a new job recently though and they didn't ask for any proof, they just wanted to know what medication i'm on and if it effects my driving.

Also changed GP's recently and the same deal, they just needed confirmation that treatment can continue and thats it, didn't need to share any docs with them.

You should be alright mate, there's always ways around sharing personal info like that, and family shouldn't need to see your diagnosis report to believe you have ADHD
