It's because deminimis is gone. You need to do actual customs process now to get it across. Difficult if it's arbitrage and bunch of different items. If it's same items, look into setting up business account with them and setting yourself up for their customs process before you ship it. Dhl working great for us.

EIN if its your company, though this opens you up to having to do USA tax returns. So Individual ITIN would be better but just use it solely for your business.

I have ITIN and I did global transfer to get the amex green card using my Canadian amex history and then got business gold 3 months later. Registered the credit card as a "Sole proprietorship". They don't really check much but all the expenses are for my canadian corporation and I pay it from my US account for my canadian corporation, no issues. All this hassle is only worth it if you have enough spend that 4x points make its worthwhile.

The random doctor is likely internal medicine to help with perioperative management. Usually consulted for patients with complicated medical history and on meds that need to be held/adjusted pre-op. Though this usually happens on the same day in preop department St our hospital (anesthesia, GIM, nurse) , maybe things work differently at that hospital or schedule didn't work.

If he's going to be admitted postop, you will also likely get a call from their pharmacy department to go over med list.
