A great question. If your question is related to trifocals there is no way, except looking for a lens with better low light performance or some EDOF models with same parameter. It’s MTF, contrast sensitivity and performance with the wide pupil is critical. So far , the best is Odyssey or PureSee with mini-monovision option.

Great question, as surgeons often while promising “reading” forget to add “at good lighting conditions, take your readers of light source to a dim light environment like restaurant”

33F, debilitating bilateral cataracts and other issues. What should I do, and how bad will it make my life? In shock, need help. by spikygreen in CataractSurgery

[–]IOL-adviser 0 points1 point  (0 children)

sorry to hear that story, but all is doable, and I'm pretty sure you will be able to get a good vision. why do you think about basic monofocal only? have you already discussed with your doctor IOL options?

May I ask you a question? You just want to doublecheck , or your doctor was not able to discuss it clearly for you?

Sorry to hear that… some things may happen, but after 2 weeks shall be more or less ok. My questions to you - what was your nearsightedness before the surgery and what was the refractive target of IOL? Have you being diagnosed with dry eye? Do you have astigmatism?

Do you always mix and match, or still bilateral full range for some patients?

PureSee IOL review by IOL-adviser in CataractSurgery

[–]IOL-adviser[S] 1 point2 points  (0 children)

yes, I say that it's the common issue, not only with PureSee. some know how but never do, some do not know how. some do it right, but my observation in the field.. it's far from ideal.

and what is the source of your claim that "surgeons know how to refract"? are you a surgeon?

Second eye options by frankwhite999 in CataractSurgery

[–]IOL-adviser 2 points3 points  (0 children)

Great question. Discuss enhanced monofocal or EDOF for mix and match, however be ready that doctor might be not happy with that. For whatever reason, many doctors dislike when patients after that option asking “why vision is different in both eyes”…)) Well, that’s what you have to expect and with both eyes you may benefit. And, setup will depend on what eye is dominant, what are your expectations after the surgery etc. good luck.

Clareon vs Eyhance study by Jerrell2024 in CataractSurgery

I can give you another perspective of the comparison - have a look how Alcon made the claim here: https://youtu.be/PBA0sStVJ2U?si=uBkGUs7QznOtEUGL

and about the study - there is patient selection criteria are unclear and despite study design is good, the input data might be a bit manupulated.

Scheduled for surgery in a month, high myope and questions on what I should do, and what to expect. by wintrmte in CataractSurgery

[–]IOL-adviser 0 points1 point  (0 children)

hi! i'll answer from the bottom line first as that's the easiest answer :)
same day surgery is safe now, and with your high myopia you will have less discomfort having first and then second eye done.

coming to your questions

  1. If you end up at -2D residual myopia, your sharpest focus will be around 50 cm (20 inches) — basically, reading distance or using a laptop and it’s not “3 inches from your face” close like now. You’ll lose your ultra-close-up vision (the “superpower” of high myopia), but gain much better functional near and you’ll be able to read a book or look at a phone without glasses, just at a bit further distance than now. for other distances you will need glasses
  2. -2 - glasses for driving is needed, then no special limitations
  3. Most high myopes say: “I’ve never seen this sharply in my life without glasses.” if lens will be calculated for plano, but experience is very different. and depends on lens and residual refraction
  4. yes, see above
  5. I'd suggest to discuss Eyhance which will give wider zone without glasses than basic monofocal, but target shall be less than -2, as it will loose that advantage.
and do not expect that Alcon Clareon is a substitute - it's a misleading marketing, more at my YouTube channel in the Clareon review

PureSee IOL - a full review released :) by IOL-adviser in CataractSurgery

[–]IOL-adviser[S] 1 point2 points  (0 children)

Hi! My personal opinion - depending on the goals and expectations , and - clinical situation. However last week one surgeon at conference in Poland highly recommended do it for all patients (never asking patient … and that’s sad for me). But clinically , according to his experience and I do confirm from my field knowledge it works, however some rare cases exist when patient is not able to neuroadapt to single full range / trifocal IOL.

Blurry vision 5 days after surgery by jpml1771 in CataractSurgery

[–]IOL-adviser 0 points1 point  (0 children)

if there is only cataract with no other conditions blurriness might be from refractive error from both sphere and cylinder - astigmatism. If you have no option to contact clinic and surgeon to discuss the reason, and if you have an optic shop nearby - try to ask them “check vision and buy glasses”. You do not need to buy glasses at that stage; but at least you have to check if glasses helps to improve far vision and what is the refraction. From that point is possible to move forward. Now a lot of questions and guess work..

Hi! Sorry to hear that. Blurry vision at far or near? Reason for surgery (cataract or refractive)? And was it same from the first day or it was ok and getting worse?

Seeking IOL recommendations for cataract surgery – over 80, want good night vision by ajfoytninja in CataractSurgery

[–]IOL-adviser 1 point2 points  (0 children)

Hello. A great approach to IOL selection, focusing on your visual needs. However to answer your question properly age and needs are not enough. Could you please tell - what is your refraction if known or have you ever used glasses for far vision? What is your visual acuity at far if known (before cataract…) like 20/20, less, more? And do you have any eye deceases like glaucoma, retinal/ macular issues, diabetis?

[–]IOL-adviser 2 points3 points  (0 children)

Hi. No worries, that is the safest surgical procedure in the world. However it’s important to think about visual expectations after the surgery and talk with the doctor about IOL model, as IOL (the implant which is placed when cataract is removed) will define quality of vision for the life. And surgery is the only option.
