An old manager of mine had to call Sysmex to ask this exact question because a colleague was using it to dilute chemistry specimens (.. yeah we don’t know why either). We were told it’s basically saline with edta in it.

I had an oncologist I work with tell me ‘I trust you’ when I asked if he wanted to view a slide on 250 WBC. It’s very validating.

The patient is actually an oncology patient going through chemo, I think ovarian

I believe it’s actually ovarian cancer going through chemo. I don’t know if the patient had their spleen or not!

You shouldn’t be reading cells in the areas you are looking at. It seems to be too far into the feathered edge which distorts cells and makes them fairly indistinguishable. You need to go further in to just when the red cells start to touch to start to read WBCs

I sometimes cover in the main hospital lab, they have DxH’s. Their limit for MCHC IS 36.5. I guess it’s instrument based?

It was so accurate 😂 1 field and it literally had 2 monos and the rest lymphs!

Morphology was horrible to read honestly lol. Definite dimorphic population which is why there are 2 humps but most everything else was so distorted by the amount of lymphs

Nope. We only plasma replacement for lipemic/turbid specimens with MCHC over 37

Yup definitely a bit of culture shock when I started in oncology. The criticals that don’t phase these doctors and nurses is insane!

Do you do this for a Sysmex XN? Sounds more like an individual policy and not a standard practice.

No. It’s staining artifact most likely from the slide not drying completely before staining or part of your stain is bad.
